Behavioral health concerns can affect your physical health, relationships, work, school, sleep, and daily life. Our integrated model helps connect your mental health support with the provider who knows your overall health history.

1. Talk with your provider

Start by talking with your Bellaire or East Jordan Family Health Center provider. They can help connect you with integrated behavioral health support.

2. Create a focused care plan

Your counselor and primary care provider can work together on a plan that considers your mental, emotional, and physical health needs.

3. Build skills and support

Visits may focus on coping skills, behavior change, substance use support, stress management, or connecting you with additional resources.

What about current therapy patients?

If you are currently in traditional therapy, your care team will help guide the transition into the integrated program. You will not be left to navigate the change alone.

The goal is to keep your care connected, focused, and supportive as behavioral health becomes more closely aligned with your primary care team.

Additional care options

Some patients may benefit from additional or ongoing therapy services outside of the Health Center. Helpful options may include local outpatient providers, Psychology Today’s therapist directory, and your insurance company’s provider list.

Ask your insurance carrier about covered behavioral health providers and referral requirements.
