Care Centered Around You

Bellaire and East Jordan Family Health Centers are recognized as Patient-Centered Medical Homes.

A Patient-Centered Medical Home is not a building or a place to stay. It is a team-based approach to primary care that places patients at the center of their healthcare.

Your primary care provider works alongside nurses, medical assistants, care managers, clinical pharmacists, behavioral health professionals and other members of your care team to help coordinate your care and support your overall health.

What This Means for Our Patients

Coordinated care: We help coordinate tests, referrals, medications, specialty care and follow-up services.
