Community Health Quality Recognition (CHQR)
The Community Health Quality Recognition (CHQR) badges recognize Health Center Program awardees and look-alikes (LALs) that have made notable quality improvement achievements in the areas of access, quality, health equity, health information technology, and COVID-19 public health emergency response for the most recent UDS reporting period.

We are honored to receive these awards!

Health Center Quality Leaders achieve the best overall clinical quality measure (CQM) performance among all health centers. They are recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), or Bronze top (21-30%).

Advancing Health Information Technology (HIT) for Quality: Recognizes health centers that meet all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care.

Health Disparities Reducer: Health centers must Qualify for the Access Enhancer badge and meet at least one of these: improve by at least 10% in low birth weight, hypertension control, or uncontrolled diabetes CQMs or meet the following targets for all racial or ethnic groups they served within the latest reporting year.

Access Enhancer: Health centers must achieve at least one of these: Earn at least one HCQL or NQL badge, improve by at least 15% in one or more CQMs in back-to-back reporting years. They must also increase by at least 5% in back-to-back reporting years (including Total patients or Patients receiving mental health, substance use disorder, vision, dental, or enabling services.)

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others.
It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts.
The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
NCQA is a private, nonprofit organization dedicated to improving health care quality. NCQA accredits and certifies a wide range of health care organizations. It also recognizes clinicians and practices in key areas of performance. NCQA’s Healthcare Effectiveness Data and Information Set (HEDIS®) is the most widely used performance measurement tool in health care. At #NextGenUGM23, they awarded the Better Outcomes Quality Improvement Contest’s Most Overall Improvement Award to East Jordan Family Health Center.
