Before visiting, please keep these important reminders in mind.
- Patients are seen by appointment only. Walk-ins are not accepted.
- Masks are required for patients and visitors with upper respiratory symptoms, even with a negative COVID test.
- Our health center is a fragrance-free space. Please avoid perfumes, scented lotions, and strong fragrances.

We’re excited to welcome new patients from Antrim and Charlevoix Counties. A few steps help us prepare for your first appointment and coordinate your care safely.
Before your first visit
- New patients must reside in Antrim or Charlevoix County, full-time or part-time.
- Complete and return the New Patient Packet and medical record release forms within one week.
- Bring all prescription and over-the-counter medications to your first visit.
- A new patient appointment does not guarantee that medications will be prescribed.

Three simple ways to help your visit go smoothly
1. Bring what we need
Please bring your insurance card, photo ID, medication list or bottles, and any completed forms.
2. Keep your appointment
Your appointment reserves time with your provider. Please give 48–72 hours’ notice if you need to reschedule.
3. Use your care team
Ask questions, update your information, and use the Patient Portal when available for routine communication.

We bill insurance on your behalf and participate with many commercial insurance carriers, Medicare, Medicaid qualified health plans, Tricare, and more.

- You are responsible for deductibles, co-pays, and balances not covered by insurance.

Need Help After Hours?
When you are ill and our offices are closed, call the after-hours nurse line for guidance.
- After-hours nurse line: 616-391-9932.
- A registered nurse can assist with health concerns.
- The nurse line cannot answer billing questions or refill prescriptions.
Update Your PCP
Your Primary Care Provider listed with insurance should match the provider you actually see. This helps with referrals, records, and coverage.
- Contact your insurance plan to verify your listed PCP.
- Update your PCP after insurance or location changes.
- Call us if you need help knowing what to ask.

Why Updating Your Primary Care Provider Matters
Ensuring Continuity of Care & Accurate Health Records
Choosing the right Primary Care Provider, or PCP, is an important part of managing your health. Your PCP is your main point of contact for routine checkups, chronic disease management, preventive care, and referrals to specialists.
If your insurance lists a PCP you no longer see, your medical records, referrals, and important health information may not reach the right provider. This can delay care or create confusion when you need services.
Why should you update your PCP?
Your PCP is your partner in health. If your insurance lists the wrong PCP, records and referrals may not reach the provider caring for you.
When should you update your PCP?
- You recently moved or are new to the area.
- Your insurance plan changed.
- Your current PCP is no longer in-network.
- You are now receiving care at Bellaire or East Jordan Family Health Center.
What should you consider?
- Choose a provider who fits your health needs.
- Confirm the provider is in-network with your insurance.
- Transfer your medical records.
- Share medications, diagnoses, and ongoing concerns at your first visit.

Appointment Changes & Cancellations
Your health is our priority. When you schedule an appointment, that time is reserved specifically for you and allows your provider and care team to focus on your individual needs.
We understand that unexpected situations can happen. If you need to cancel or reschedule your appointment, please notify us 48–72 hours in advance whenever possible.
Providing advance notice helps us:
- Offer the appointment to another patient in need
With enough notice, we may be able to offer that appointment time to another patient who is waiting for care. - Maintain efficient scheduling and reduce wait times
Short-notice cancellations can disrupt the schedule and may lead to longer wait times for other patients, including when you need to reschedule.

New Patient Information – What to Expect
We’re excited to welcome new patients to Bellaire & East Jordan Family Health Centers! As a trusted healthcare home for Antrim & Charlevoix counties, we ensure that every new patient receives comprehensive, patient-centered care.
How to Get Started
✅ Residency Requirement: New patients must reside in Antrim or Charlevoix County (full-time or part-time). If they live outside the area, their application may be reviewed by our Medical Director.
✅ Complete & Return Forms Promptly: Patients must complete a New Patient Packet and return all forms, including medical record release forms, within one week. Without these, we cannot request your medical history from previous providers, which is critical for providing high-quality care. If forms are not returned within a week, your new patient appointment may be rescheduled.
✅ Records Are Essential for Quality Care: New patient appointments are scheduled at least 30 days out to allow time for our office to obtain medical and behavioral health records. Having your full medical history ensures a smooth transition and helps us provide the best possible care.
✅ Bring Your Medications: Patients should bring all prescription and over-the-counter medications they are taking to their first appointment to ensure accurate medication management.
✅ Medication Policy: A new patient appointment does not guarantee that medications will be prescribed. Our medical team will work with you to create a personalized treatment plan, including medications as appropriate.
