Tips to Transfer Your Prescription

  1. Gather Your Prescription List: 
    You’ll need the name, dosage, and Rx# of each medication.
  2. Gather Your Health & Insurance Information
  3. Fill out a “Transfer Request Form”:
    Available online or through our pharmacy.
    Drop off the completed form to help us get started on your patient profile and transfer.
  4. Let Us Handle The Rest:
    We’ll handle the transfer from your current pharmacy.
  5. Remember To Plan Ahead:
    Plan a few days in advance to ensure a smooth process.

Insurance and Prescription Cost Support

Our pharmacies participate with most commercial insurance plans, Medicare Part D, and Medicaid plans. We also routinely review GoodRx pricing to help keep our prescription costs competitive.

Limited or No Prescription Coverage? We Can Help.

For established patients of Bellaire and East Jordan Family Health Centers who do not have prescription drug coverage, financial assistance may be available through our Discount Program.

This program may offer low-cost prescription co-pays, often under $20 per prescription for eligible patients. This support is made possible through the 340B Discount Program, which is unique to Federally Qualified Health Centers and is not available at most commercial pharmacies.

If you do not have prescription insurance, have limited coverage, or are unable to afford your medication, please talk with our pharmacy team. We may be able to help.
