Complimentary athletic physicals will be available courtesy of the East Jordan Family Health Center to students of Antrim and Charlevoix Counties.
You MUST schedule with the East Jordan Family Health Center before July 5.

Required Items
- You must have the MHSAA physical form filled out and signed by parent/ guardian with you to receive the physical.
- The Sports Physical Permission Form must be signed by a parent/ guardian.
- Both forms are available on this page or at our office.
The appointment will be canceled due to incomplete documents.
