2024-25 Sports Physicals
The most frequently requested and downloaded form is the Student-Athlete Physical Exam / Medical History Form.
Sports Physicals: For the 2023-24 school year, an MHSAA pre-participation sports physical is required for all students who participate in sports. A physical examination must be completed on or after April 15, 2023 to be considered a valid physical for 2023-24. The MHSAA Health Questionnaire will not be in use. Please be sure to inform all parents and students – especially those who will begin middle school next fall – of the April 15 date. The MHSAA two-page physical form has not changed and remains on MHSAA.com.

2024 COMPLIMENTARY ATHLETIC PHYSICALS
Complimentary athletic physicals will be available courtesy of the East Jordan Family Health Center to students of Antrim and Charlevoix Counties.
You MUST schedule with the East Jordan Family Health Center before July 5.
SPACE IS LIMITED.
Walk-ins will not be accepted.
When: July 11, 8:00 am – 12:00 pm
Where: Bellaire and East Jordan Family Health Centers
Required Items
- You must have the MHSAA physical form filled out and signed by parent/ guardian with you to receive the physical.
- The Sports Physical Permission Form must be signed by a parent/ guardian.
- Both forms are available on this page or at our office.
The appointment will be canceled due to incomplete documents.
