Dear Patients,
We have missed seeing you in person over the last couple of months and thought this would be a great time to update you on availability of appointments at our Health Center!
We are OPEN for both telehealth and in-person visits, although we prefer you utilize a telehealth visit or a telephone visit for the next few weeks as possible. Telephone and telehealth visits keep you safe and healthy, as well as protect our community and staff from further spread of COVID-19.
Current hours at both locations: Monday – Friday 9 am – 5 pm and Saturday 9 am – 1 pm.
- Please call for an appointment first. WALK IN PATIENTS will be re-directed to make an appointment for services.

- Here are a few screening questions we will ask you before you enter our building:
- Do you have a fever? Do you have a cough?
- Are you experiencing respiratory symptoms?
- Have you been in direct contact with someone with COVID-19?
Depending on your answer to these questions, we may convert your visit to a telehealth visit. If you arrive on premise with any respiratory symptoms, we will ask you to kindly wait in your car in the parking lot and then direct you to the appropriate space in our building for your appointment.
We require all patients during their appointments the following:
- We will provide hand sanitizer. We ask you to use it when you enter. We require that you bring a mask with you to your appointment and enter wearing it. Our staff will be wearing masks as well.
- You will notice that our waiting rooms have fewer chairs and no longer contain magazines and children’s toys. This is because these items are more difficult to disinfect.
- We are limiting companions during your appointment. We ask that you come alone unless a caregiver is necessary for the patient’s safety.
- Our in-person appointment schedule will be lighter than usual to ensure we have fewer patients in the building at any time.
Laboratory Services: We have a partnership with LabCorp to provide laboratory services. Please make lab service appointments online at www.labcorp.com/labs-and-appointments or by calling the Bellaire lab at (231)533-9315 or East Jordan lab at (231) 536-2930.
COVID-19 Testing: We are also providing COVID-19 testing by appointment. Please call (231) 222-2320 to discuss your testing needs or schedule a coronavirus test.
