The Health Department of Northwest Michigan is working with state and local agencies, area hospitals, pharmacies, and other partners to implement the MDHHS plan for a phased approach to distribution. Because quantities of the vaccine are limited – and will be for the foreseeable future – this approach follows CDC guidelines and prioritizes the highest-risk populations. While timing of distribution is not yet certain and depends, largely, on vaccine supplies, we are currently vaccinating individuals included in Phase 1A. Individuals in Phases 1B and 1C will be prioritized in early 2021, while individuals in Phase 2 can expect to be eligible by mid-2021.

Individuals in Phases 1A and 1B can complete the COVID-19 Vaccination Information Form to notify the health department of their interest in receiving the vaccination. We can then follow up with each organization once vaccine is available and we are ready to schedule appointments.

The Bellaire and East Jordan Family Health Center is experiencing issues with our internet systems. This has also impacted our East Jordan location phone systems. We are working to resolve this issue with Charter. Thank you for your patience.

The Patient Portal will be unavailable October 18-20, 2024 due to scheduled maintenance. We apologize for the inconvenience.
