The East Jordan Family Health Center was notified on March 30, 2021 by a business associate, Capture Rx, that patient protected health information contained within their system had been accessed without authorization. Capture Rx is a 340B third party administrator that East Jordan Family Health Center contracts with to increase 340B eligibility in our local pharmacies. Any individual health center patients involved in the data breach will be notified directly.
The data accessed in the breach included personal information such as name, date of birth, and prescription information. The data DID NOT include any credit card information, purchasing information, or social security numbers of patients. Capture Rx has taken steps to repair the impacted software including changes to ensure this breach does not happen again.
