Dental Support for Eligible Patients
Bellaire and East Jordan Family Health Centers pay the $50 Northern Dental Plan membership fee for eligible patients. This includes x-rays, an exam, a treatment plan, and access to discounted dental services.
How to Apply
Step 1
Complete a Discount Program application.
Step 2
Include proof of household income for all family members.
Step 3
Return the completed paperwork to either Health Center location.
Accepted Income Documents
Accepted documents may include:
- Recent paycheck stubs
- Social Security award letters
- Pension or unemployment statements
- Child support documentation
- Most recent federal tax return – W-2 forms are not accepted
