At-Home COVID-19 Test Kits
As a Federally Qualified Health Center, East Jordan Family Health Center is able to distribute these At-Home COVID-19 Test Kits for no charge through a program from the Health Resources and Services Administration.

Based on availability, there may be limits placed on the frequency and quantity of tests that are distributed per family.

Follow the instructions that come with your kit very carefully. For more info about self-testing and how-to videos, you can use the QR code in the graphic shown here. You can also access this information at:

If You Test Positive:
If you test positive, follow all recommended CDC guidelines. The most current guidelines are listed in this flyer at the date of publishing. We will update as frequently as needed.

If you test positive, contact your healthcare provider for information on possible treatment options and medical advice. You should also contact your healthcare provider if you are having worsening symptoms.
