East Jordan Family Health Center is committed to providing quality patient care. As part of this commitment, we work continuously to improve the health outcomes of our patients.
We do this by monitoring several clinical performance measures that are part of our Health Care Plan and taking action to improve our performance on those measures. These performance measures provide us with the opportunity to establish quality and performance goals for the Health Center and our patient populations and to track our progress toward those goals. The measures:
- Address priority health conditions and cover all life stages
- Focus on preventative health care
- Are derived from evidence-based clinical guidelines
Our clinical performance measures document services related to good long term health outcomes. It is known that people who receive timely routine and preventative care are more likely to have improved health status. Thus, by increasing the proportion of health center patients who receive timely preventative care and routine acute and chronic care, we can expect improved health status of our patient population in the future.
