    The Bellaire and East Jordan Family Health Centers are proud to be the medical home to over 8,000 of our community members. All of our medical providers are Board Certified professionals dedicated to keeping you healthy with the latest knowledge and technology.

    Billing/Patient Accounts

    The Health Center bills all insurances on your behalf for services you receive and we participate with many commercial insurance carriers including Blue Cross Blue Shield of Michigan, Blue Care Network, Priority Health and Tricare, as well as plans such as Medicare, and other Medicaid qualified health plans. You are responsible for any amounts that are not covered by your insurance including deductible and co-pay amounts. Please make sure that we receive a current copy of your insurance card at each appointment so that the billing can be processed accurately. Your insurance company is the expert about your policy, and you should contact them directly to answer your questions about the services they cover for you and what your cost share amounts are.

    Your Primary Care Provider (PCP) is your partner in health. But if your insurance lists the wrong PCP, your medical records and referrals may not reach the right provider—delaying care.

    Location Changes – New to the area? Choosing a nearby provider ensures you have easier access to care.

    Insurance Plan Changes – If your new insurance doesn’t cover your current PCP, updating to an in-network provider prevents unexpected costs.

    What to Consider When Changing Your PCP

      • Research Providers – Find a doctor that fits your health needs and is in-network with your insurance.
      • Transfer Medical Records – Ensure a smooth transition by sharing your history with your new PCP.

    We’re excited to welcome new patients to Bellaire & East Jordan Family Health Centers! As a trusted healthcare home for Antrim & Charlevoix counties, we ensure that every new patient receives comprehensive, patient-centered care.

    How to Get Started

    ✅ Residency Requirement: New patients must reside in Antrim or Charlevoix County (full-time or part-time). If they live outside the area, their application may be reviewed by our Medical Director.

    ✅ Complete & Return Forms Promptly: Patients must complete a New Patient Packet and return all forms, including medical record release forms, within one week. Without these, we cannot request your medical history from previous providers, which is critical for providing high-quality care. If forms are not returned within a week, your new patient appointment may be rescheduled.

    ✅ Records Are Essential for Quality Care: New patient appointments are scheduled at least 30 days out to allow time for our office to obtain medical and behavioral health records. Having your full medical history ensures a smooth transition and helps us provide the best possible care.

    ✅ Bring Your Medications: Patients should bring all prescription and over-the-counter medications they are taking to their first appointment to ensure accurate medication management.

    ✅ Medication Policy: A new patient appointment does not guarantee that medications will be prescribed. Our medical team will work with you to create a personalized treatment plan, including medications as appropriate.

    However, we understand that unforeseen circumstances may arise. If you need to reschedule your appointment, we kindly request that you notify us at least 48-72 hours in advance.

    This advance notice allows us to:

    • Offer the appointment slot to another patient in need: By providing sufficient notice, we can offer the time slot to another patient who may be waiting for an appointment. This helps ensure everyone in our community can access the care they need promptly.
    • Maintain efficient scheduling and minimize wait times: When cancellations occur with short notice, it can disrupt our schedule and lead to longer wait times for all patients, including yourself when you reschedule.
