Sliding Fee Discount Program

The Health Center has developed a Fee Discount Program for patients and their families based on income and family size. This program may provide you with a substantial discount on the following services: On-site office visits, on-site procedures, on-site X-ray services and assistance with prescription costs.

Our Patient Resource Coordinator is available to meet with those uninsured or underinsured to help determine eligibility for health care coverage and prescription assistance programs. We will help you with the application processes for these programs so that you have access to affordable, quality healthcare.

We believe that everyone has the right to affordable health care.

What is it?

The Fee Discount Program provides reduced fees for the following services:

  • On-site Medical Visits
  • On-site Procedures
  • On-site Behavioral Health Visits
  • On-site X-ray Services
  • Assistance with prescription costs at our East Jordan & Bellaire on-site Pharmacies
  • Laboratory Services (through arrangement with LabCorp)
  • Optometric Exams (through arrangement with Dr. Voci)
  • Dental Services (through Dental Clinics North)*
    • East Jordan Family Health Center will pay the $50 membership fee of Northern Dental Plan (NDP) for eligible sliding fee patients. The membership includes x-rays, an exam, a treatment plan, and access to the NDP sliding fee schedule for services.

Who is it for?

All patients and families who are established with Health Center providers and who qualify based on income and family size.

How do I Apply?

1. Complete the application below.

2. Provide evidence of your present income for all family members. Examples of income include:

  • Most recent paycheck stubs
  • Child support ordered
  • Social Security Award Letter
  • Land contract payments received
  • Monthly pension statement
  • Unemployment benefit determination statement
  • Worker’s Compensation checks
  • Most recent Federal tax return – W-2’s not accepted

3. Mail or return to the address shown on the back of the application.
